2 Why is there no sound coming from one or both earphones?
I cannot hear any sound in one or both of the earphones, I can only hear the game noise and not chat, or I can only hear chat and not the game.

The volume for the headset may be set to the lowest setting. The adapter adjusts both the chat and game audio levels, check to see both are turned up. Make sure the Xbox controller is charged, and that the audio cable and adapter are fully connected.
