This could be a result of a few things. If they hear you okay when you're not on speaker phone, chances are this is an issue that just affects that microphone. Speakerphone usually makes use of the microphone in the earpiece speaker if I recall correctly. It may be there is some crud stuck in there. It happens frequently especially if you talk on the phone frequently or keep your phone in your pocket.
Otherwise a lot of that same steps will apply between audio and microphone so the iPhone No Sound page might also be helpful.
