The nice thing about the Nanos is their diminutive size. The not so nice thing is that they achieve this by soldering most everything directly to the logic board.
Having that said, you would need to replace the logic board to replace the headphone jack.
I am unaware of anyone that sells just the jack, as it requires such intricate (i.e. machine-run) soldering.

You can currently purchase a replacement earphone socket on eBay for $4.99. Search for something like "ipod nano 2nd generation ribbon cable". The earphone socket is attached to the replacement click wheel cable.
