Hi,
Unless you need a new back cover, you can simply replace the rear facing camera lens.
Here’s a link to the ifixit Samsung Galaxy Note 4 Rear Facing Camera Lens Replacement guide.
Replacement lens’ are available online. Just search for Samsung Galaxy Note 4 Rear Facing Camera Lens replacement part to get results for suppliers of the part.
